Background of the Student Background Questions that you should answer in this section about the student (other data is always helpful): • Who are you interviewing? • Where do s/he live? • Who does s/he live with? • What does s/he want to do as after graduation? • What are her/his expressed educational goals? • What are his/her expressed employment goals? • What are his/her expressed independent living goals? • What are his/her interests? • What are his/her perceived strengths? • Who is s/he close with? (relationships, children, friends, etc.) • Does s/he have a mentor in her/his field of choice? (Document the day when the data was obtained from your partner) Assessments In this tab, inform the reader of the assessments that you gave your partner, and also the findings of each assessment. Also note if these assessments match information that your partner expressed in the initial interview. (Note: document the day when each assessment was given to your partner and what data was found from each assessment.) Goals Post-Graduate Goals. Here you will enter in your post-graduate goals. You will have three total post-graduate goals. Post Graduate Employment Goal: Post Graduate Education Goal: Post Graduate Independent Living Goal: Annual Post-Graduate Goals. Here you will enter your post-graduate annual goals. You will have four total annual goals. Know that one post-graduate goal will have two annual goals. Identify the post-graduate goal that the annual goal supports. Example: Post Graduate Employment Goal: Annual Goal #1 Annual Goal #2 Post Graduate Education Goal: Annual Goal #1 Post Graduate Independent Living Goal: Annual Goal #1: (Note, Chelsea and I should be able to see if these goals are reasonable based on data obtained in the background and assessments). Future Recommendations and Reflection This is the second section of your paper. Here, you want to recommend steps your partner can take to ensure success of the transition goals. Assessments In this section, you will list assessments that you will use to determine progress. Some of these can be class related assessments, formal assessments, or interviews. Please recommend at least three assessments. Work Samples You will research work samples. Be sure to define what is a work sample. Work samples can assist you in understanding your partner’s performance and progress on annual goals and post-graduate goals. In this section, you will research 3 work samples. You will tell us why these work samples are important and how each sample will support an annual goal. (Hint: work samples do not need to be complicated. If you are working with a partner who wants to buy a car, you create an activity that will assist your partner in collecting documents (pay stub, credit score, budget) necessary to buy a car. Get creative if you want!) Community Opportunities In this section, you can do research to recommend community opportunities for your partner. For example, there is a Virginia Chapter of Division of Career Development and Transition you can recommend your partner. There are also EBD chapters under CEC. In this section research 1 community opportunity that matches with your partner’s interest, and two professional development opportunities. PD opportunities can be a conference that may be close, or a chapter of a professional educational group that your partner is interested in working in after graduate school. Self-Reflection In this section, you will reflect on this process. Some questions you can answer are: • How was it working with a partner in an online class? • How difficult was it to work with someone who has a different learning process than you? • How did this process make you reflect on interagency collaboration?   References You need to cite your interviews with your partner. You need to cite the assessments used. You will need to cite definitions of work sample. You will need to cite any work samples that you did not create. You will need to cite any professional affiliations that you recommend to your partner.
